How your PhilHealth premium is computed

Under the Universal Health Care Act (RA 11223), the 2026 premium is 5% of your monthly basic salary, but only within an income floor and ceiling:

Earn ₱10,000 or less

Fixed ₱500

flat minimum premium

Earn ₱10,000–₱100,000

5% of salary

Earn ₱100,000 or more

Fixed ₱5,000

flat maximum premium

If you're employed, the premium is split 50/50 — you pay half, your employer pays the other half. Voluntary, self-employed and OFW members pay the full premium themselves.

Worked example: ₱30,000/month employee

Monthly basic salary₱30,000
Total premium — 5% of ₱30,000₱1,500
Your share (half)₱750
Employer share (half)₱750
Only ₱750 is deducted from your payslip. PhilHealth gives you free access to hospital coverage, the Konsulta primary-care package, and case-rate subsidies for surgery and confinement.

PhilHealth questions Filipinos actually ask

"Do I pay the full premium or just half?"

If you're employed, just half — your employer pays the rest. Only voluntary, self-employed and OFW members pay the full 5% premium themselves.

"My salary went up but my PhilHealth didn't — why?"

Because of the ₱100,000 ceiling. Once you earn that or more, your premium is frozen at the maximum ₱5,000 (₱2,500 as the employee), no matter how high your pay climbs.

"Is it worth it if I rarely get sick?"

PhilHealth isn't just for confinement. It covers outpatient consultations and lab tests under Konsulta, dialysis, chemotherapy, and case-rate subsidies that knock thousands of pesos off a hospital bill — and it covers your declared dependents too.
